Fast Track Urgent Care Provides Notice of Vendor’s Data Security Incident

Fast Track Urgent Care (“Fast Track”) is an urgent care clinic located in the state of Florida. Fast Track is providing notice that one of its vendors, PracticeMax Inc. (“PracticeMax”), experienced a cybersecurity incident that may involve the personal and protected health information of some Fast Track’s patients. Fast Track sincerely regrets any inconvenience that this incident may cause and remains dedicated to protecting all personal and health information.

Although Fast Track has no evidence that anyone’s personal information has been misused, Fast Track is providing details about the events, steps taken in response, and resources available to individuals to protect against the potential misuse of their information.  Out of an abundance of caution, PracticeMax, on behalf of Fast Track, provided individual notifications to potentially impacted persons.  However, Fast Track is providing this substitute notice because there are missing or outdated mailing addresses for some Fast Track patients. 

What Happened:

This matter involves a data security incident involving one of Fast Track’s vendors known as PracticeMax.  PracticeMax is Fast Track’s billing vendor which collects and processes payments from Fast Track’s patients.   This incident did not affect Fast Track’s internal computer systems.

On May 1, 2021, PracticeMax reportedly discovered that an unauthorized individual had gained access to its computer systems. PracticeMax first notified Fast Track of this incident on May 10, 2021.  However, at that time, PracticeMax could not confirm whether or not personal information of any Fast Track patients had been impacted.  On February 14, 2022, PracticeMax informed Fast Track for the first time that Fast Track’s customer and patient data may have been impacted as a result of the incident.  However, PracticeMax’s investigation was still ongoing at that time to determine whether Fast Track customer and patient data had been subject to unauthorized access as a result of the incident.   Finally, on June 6, 2022, PracticeMax confirmed that an unauthorized individual was able to access Fast Track’s customer and patient data.

What Information Was Involved:

PracticeMax’s investigation confirmed that certain Fast Track customer and patient information may have been subject to unauthorized access.  This information included one or more of the following data elements: individual’s name, date of birth, Social Security number, passport number, driver’s license or state identification number, treatment and/or diagnosis information, health insurance information, financial information, and/or health information. Please note that not all data elements were present for every customer or patient of Fast Track.

 

Complimentary Credit Monitoring:

PracticeMax is offering complimentary credit monitoring to all affected individuals.  In addition. PracticeMax has reportedly taken several corrective actions to remediate the security incident, prevent a further security incident, and mitigate the effects of the security incident. PracticeMax has reviewed their existing policies and procedures and implemented additional safeguards to further their already stringent security policies and procedures and to secure the information in their systems.

Fast Track recommends that its patients enroll in the complimentary credit monitoring services offered by PracticeMax and follow the recommendations contained within the notification letter to ensure their information is protected.
