Requesting Personal Medical Records
Patients are welcome to request records by calling, using their patient portal, coming into any of our convenient locations, emailing a signed request form to medicalrecords@fasttrackurgentcare.com or faxing a signed request form to 813-749-8370.
Requesting Medical Records for a Mutual Patient (Primary Care Providers or Specialists)
For primary care or specialist requests for medical records, please email your request to medicalrecords@fasttrackurgentcare.com or send your request via fax to 813-749-8370.
3rd Party Requests for Medical Records
TGH Urgent care powered by Fast Track will now be fulfilling billing record requests electronically via ChartSwap, www.ChartSwap.com.
ChartSwap is a HIPAA compliant platform launched to facilitate electronic medical and billing record exchange between medical providers and law firms, and other requesting parties. It’s free to register and requestors can use ChartSwap to request, track, pay for and download records.
To submit requests via ChartSwap.com use the following steps.
- Register at http://www.chartswap.com/register as a Record Requestor;
- Sign in and Search for a Provider, Enter Request Details, then Upload Supporting Documents;
- Once your request has been reviewed and records are available, you will receive a notification and invoice, which you can pay with a check or credit or debit card.
If a request has been mailed/faxed, you will receive a secure email notification when your request has been updated. Simply follow the link in the invite to register and view invoices, records or messages related to your request. If you do not wish to register, please contact ChartSwap or the provider and the request will be fulfilled outside of ChartSwap.
